INDEPENDENT REVIEW FOR BOARDS AND EXECUTIVE TEAMS

Decision Integrity Review for critical decisions

When a decision depends on information that is incomplete, contradictory, or difficult to verify, the risk also lies in how that information is interpreted. A Decision Integrity Review examines the informational basis of the choice, helping those accountable for it identify which conclusions the evidence supports, which depend on assumptions, and which checks deserve priority.

When to involve the practice

The service supports boards, executive teams, risk functions, and advisors assessing decisions affected by cyber threats, technology dependencies, AI risk, or geopolitical developments. It can be commissioned before a plan is approved, during a crisis in which accounts diverge, or when the organization needs to reconstruct what could have been known at the time of an earlier decision.

The engagement defines the question, the intended reader, the deadline, and the decision authority involved, so the review addresses a specific organizational need.

How the review is developed

The analysis reconstructs the provenance and sequence of the available information, assesses source reliability, and compares competing interpretations. It makes assumptions, gaps, and contradictions explicit, linking conclusions to their supporting materials and to the limitations that affect their use.

Examining alternatives also identifies the evidence that could change the assessment and the questions requiring further investigation. A documented fact, an analytical inference, and a conditional consequence retain distinct roles throughout the review, allowing the reader to follow how the assessment was reached.

What the client receives

Deliverables comprise an information matrix covering facts, sources, assumptions, and gaps; an options matrix describing plausible consequences and conditions for reassessment; and a decision memo that prioritizes checks and records the reasoning. Their scope is agreed around the problem and the intended decision, so the final materials can be used by the people responsible for acting on them.

An illustrative application

Suppose a supplier announces service restoration while internal teams report that important integrations remain unavailable. The review can separate what the supplier's statement establishes from what the client needs to verify before restarting its own process. This illustrative example shows why a decision to resume operations may require evidence beyond that needed to close a particular technical task, especially when several functions rely on different parts of the same service.

Starting an engagement

The initial conversation covers the decision, its general context, the deadline, and the categories of material available. Scope, sharing arrangements, and expected outputs are agreed before confidential records are transferred. The client retains responsibility for the decision, while the review provides a traceable assessment of the information on which it rests.
